How to Create an Online Order Form
Build an order form with product selection, quantities, pricing, and integrated payment collection. Perfect for small businesses.
Before you start
- A free Formet account
- A PhonePe business account (for payments)
Start with the Order Form template
From your dashboard, click "Create New Form" and search for "Order Form" in the template library. This gives you a pre-built structure with customer details, product selection, and payment fields.
Customize products and pricing
Edit the product selection field to list your actual products/services with prices. Add options for sizes, colors, or variants as dropdown or radio fields. Set up quantity selectors.
Add shipping details (if physical)
For physical products, add address fields: Street, City, State, PIN Code. Mark them as required. You can use conditional logic to show shipping fields only for physical products.
Configure payment
Add a payment field and connect your PhonePe gateway. The payment amount can be calculated automatically based on product selection and quantity. 0% platform commission.
Set up order notifications
Configure email notifications so you receive an alert for every order. Enable auto-response to send an order confirmation to the customer with their order details.

Can I add delivery charges?
Yes. Add a fixed delivery charge or make it conditional based on location or order value using calculation fields.
Can customers order multiple items?
Yes. Use repeatable sections or multiple product selection fields to allow customers to add multiple items to their order.
Is there inventory tracking?
Currently, Formet doesn't track inventory. For inventory-aware order forms, connect to Google Sheets and manage stock there.
