Good form design is invisible. It guides the user from start to finish without friction. bad design creates cognitive load and leads to abandonment.
Single vs. Multi-Column
In almost all cases, a single-column layout is superior. It creates a clear vertical path for the eye to follow. Multi-column layouts cause the eye to zigzag, increasing cognitive load.
Label Placement
